Affidavit of Support

Affidavit of Financial Support (Prise en Charge)

A financial support declaration is signed by a (prospective) student’s sponsor, guaranteeing payment of travel, living, educational, medical and repatriation expenses. You can find the minimum necessary income on the website of the Immigration Office.

This form needs to be completed in the language of the city where the student will  register :

For your convenience, you can find bilingual versions of the form hereunder to reference if you are in need of translation. These WILL NOT be accepted in the place of the single language version. DO NOT fill this out.

To have an affidavit of support legalized you can only submit the documents in person via an appointment, or by mail.
We cannot accept walk-ins.

In both cases (whether you come in person or send the form by mail), please provide the following documents as proof of solvency:

  • Proof of identity (driver’s license, US Passport, foreign passport with valid visa, green card, etc.)
  • Most recent W2 form
  • Recent letter of employment stating type and length of contract (delivered in the last 6 months)
  • Pay slips covering the last 3 months
  • Form 1040 'individual income tax return' (first two pages)
  • Bank statements covering the last 3 months
  • An acceptance letter or proof of registration for the school that the student will be attending in Belgium

Please provide us with your email address and phone number so that we can contact you in case of an incomplete file.

We reserve the right to ask for additional documents as necessary.

 

IN PERSON:

If you come in person do not sign the declaration of support form yet. Please wait until you appear in front of the visa agent.

BY MAIL:

If submitting these documents by mail, the following instructions apply to the declaration of support form:

  • The sponsor needs to date and sign the financial support declaration document,
  • Add the mention ‘read and approved’ in the language of the form
    • in French: ‘lu et approuvé’
    • in Dutch: ‘gelezen en goedgekeurd’
  • Join a notarized copy of the sponsor’s form of ID
  • Include a money order or cashier's check for the legalization fee
  • Include a pre-paid and pre-addressed return envelope so we can return the legalized document to you
  • Do not have the sponsor’s signature legalized by a notary public or an Honorary Consul.

 

    Note: We do not return the documents submitted, and we do not make copies. Please be sure that you have an original (for you to keep) and a copy (for us) available.

    The signature legalization fee is 24 USD.

     Personal checks are not accepted.

    Certified checks and money orders have to be paid to the order of:

    • "the Embassy of Belgium in Washington" if you apply at the Embassy in Washington
    • "the Consulate General of Belgium" if you apply at the Consulate General in Atlanta, Los Angeles or New York.

    Incorrect money orders or certified checks will be rejected and returned to the applicant. Processing will only begin once the proper payment is received.

    Only at the Embassy of Belgium in Washington and the Consulate General of Los Angeles may you pay with Visa or MasterCard.
    For applications sent via mail to Washington DC, the Credit Card Payment Authorization Form can be used.
